  1. To improve birth outcomes by reducing the prevalence of low birthweight infants among Medicaid-eligible women in Colorado through care coordination, nutrition and psychosocial counseling services
  2. Voluntary program for Medicaid-eligible pregnant women at risk for a low birthweight infant
  3. Addresses perinatal depression through education, universal screening and referrals for treatment when indicated
